However, experts failed to recognize the warning signs and missed the opportunity to put him on a de-radicalization program. Rudakubana had been referred to the Government's Prevent program three times but his case was closed prematurely without a full investigation. The review found that his fascination with extreme violence should have raised concerns. Security minister Dan Jarvis has raised questions about how various agencies failed to act on the warning signs.
